If you are a Mammoth patient who used the Mammoth Web Properties to access Mammoth’s "Your Medical Record" located at https://mammothhospital.org between August 9, 2020, through August 9, 2023, then you may be eligible for benefits.

If you are a Settlement Class Member and wish to receive one or more of the following settlement benefits then you must submit a claim by October 14, 2025.

Mammoth has agreed to pay for two benefits that are available to all Class Members. Class members may claim one or both of them.

CyEx Privacy Shield Pro: CyEx Privacy Shield Pro is a service that keeps your personal information safe by checking for it on the dark web. If it finds your private details there, it will alert you immediately. If fraud does occur, a dedicated case manager will help you fix the problem.

Cash Payment: All Class Members may claim a one-time $20.00 cash payment.

To start your online claim, you must login with your Unique ID and PIN. You can find your Unique ID and PIN at the top of your email or postcard notice. If you are unable to locate your ID & PIN, please contact the Settlement Administrator by emailing info@MammothHospitalPixelSettlement.com or calling 1-833-360-6895. Please request your Unique ID & PIN by providing your full name and mailing address.
